Instant death in town hall

What happened:
I had finally reached the level for crafting the M203GL and was messing around with it in the woods.
I was asking !help for how to use it. I logged out for a break and went into gmod again.
When I spawned in townhall I got instantly killed. I mean like 0.3 seconds after I spawned.


What do you need refunded: G3A3, Kevlar (No helmet), First Aid Kit (S) x2, Lockpick, P99 and M203GL. I don't suppose GL ammo includes.
In the picture you can see the attachment on the G3A3 - it should also be in the log.

Hello, I am able to confirm the following items were lost due to a Mass RDM.

Kevlar No Helmet
First Aid Kit (S) x2
Lockpick
P99

Sadly the screenshot provided is not enough proof to back up the M203GL so I am unable to verify that.
