I can CONFIRM that the above was lost. Additionally, x1 kevlar extra was lost, making the total 3 kevlar+helmet being lost.

In terms of the situation, iirc it involved a police sergeant misunderstanding the 2nd life raid rule and as soon as they respawned, they began giving information that the other PD wouldn't have known otherwise. This was cleared up with the sergeant and the deaths were voided as a result.
