I can't find the actual unban appeal format anymore

Ban Reason: Mass rdm
Unban Reason: My last unban appeals were denied I figured i didn't give it enough time so I'm back again trying to see if I can get another chance to play have matured a lot have no malicious intent and never did.

Time of Occurrence: couple years ago.

That's just the ban reason I never actually mass rdmed anyone I never had any chance to even explain anything. i figured that was already known.
