What happened: Larry Fish was rdming (mass) on swat, he recieved a punishment in game got told too take my refund to the fourms.

Initially, I did not recall the details of this incident, but after talking to @Ace Carter and sifting through the logs, I can confirm the validity of this refund request. Larry Fish used a grenade launcher and killed innocents in the process, resulting in a weapons blacklist. However, the drop on death items that were on him did not drop for some reason according to the logs. Later down the road, he was RDM'd again, in which all of the items that should have been gone the first time around, were then grabbed.
