Unban Reason: The game crashed immediately after I died on the new map, I've constantly been having issues with lag already and I'm not surprised. Garry's mod literally just self-terminated.

Video shows my game actually just freezing and doing nothing after I died, it shortly ended the process and medal didn't catch anything else because of that.

Sadly, your ban appeal has been denied. While the video you posted could have easily been a crash, it could also just as easily been you hitting alt + f4. In addition, the server is typically able to detect when you crash vs when you time out.
